How you Register
If you live in our practice area and would like to register with the practice call into the surgery and one of our receptionists will explain the process to you. Your registration will be with the practice and you can see the GP of your choice. Before registering we will need to see 2 forms of ID and one of these must show your current address.

Changing Your Details
If you change your name, address or telephone number please let us know as soon as possible. You can inform us by telephone, fax, email (use our Update Contact Details page) or in person.
Changing to Another Practice
You have the right to register with any doctor of your choice elsewhere, subject to that doctor’s acceptance. Once you have registered elsewhere we will promptly forward all of your medical records.
